Nurses, Counselors, and other interested Health Professionals

COURSE DESCRIPTION:
• This is a six-hour training aimed at providing an introduction to Case Management for the counselor, nurse, or other professional
• This course directly relates to the domains of assessment, counseling, case management, client education, and professional responsibility, with a specific substance abuse focus.

OBJECTIVES: (Behavioral Terminology)
Upon completion, the learner will be able to identify:
1. Major theories and models and principles of case management and their application with Substance using Clients
2. Appropriate case management activities, including record keeping, recovery issues, community resources, and continuum of care.
3. And establish a systematic approach to monitor the treatment plan and maintain quality of life.
4. Means to establish and maintain the continuum of care as an ongoing recovery process and develop agency networking.
5. Appropriate assessments, interventions, professionalism, and supportive measures such as case management and client education for the professional who is caring for the chemically impaired individual.
