COURSE DESCRIPTION:
• This is a six-hour training aimed at providing an introduction to Report, Record Keeping, and Defensive Documentation for the counselor, nurse, or other professional
• This course directly relates to the domains of assessment, counseling, case management, client education, and professional responsibility, with a specific substance abuse focus.

OBJECTIVES: (Behavioral Terminology)
Upon completion, the learner will be able to identify:
1. The purpose of and rationale behind Report, Record Keeping, and Defensive Documentation
2. The purpose of and rationale behind Referral and Consultation
3. Concise, clear verbiage for documentation in records and reports
4. Circumstances under which referral and/or consultation is warranted for various Substance UseDisorders and comorbidities
5. Evaluation of client records along the continuum of care
